Main Campus Transcript Request Forms [Download PDF]

If you need official transcripts and you are attending or have attended ENC as a traditional undergraduate on the main campus or graduate student in the Masters of Marriage/Family Therapy or Masters of Education program use this form.  Transcript requests must be submitted in writing with a signature.  Please click on "Transcript Request Forms" above for the actual form and directions. Please include a check or money order when mailing in your request. If you have had an official  name change since you attended ENC you must submit a copy of a legal document such as a driver's license validating that name change. Otherwise transcripts will be sent with original name. Please write LEGIBLY. Thank you. Main campus transcript questions should be directed to Yvonne Christensen (617-745-3879) transcripts@enc.edu . Cost for each transcript is $5. ($10 fee for same day requests).

Adult Studies Transcript Requests (LEAD Students) [Download PDF]

Students who need transcripts of courses taken as part of the Adult Studies program (formerly LEAD) need to fill out the Adult Studies transcript request form.  Adult Studies transcript questions should be directed to 617-745-6814.  Cost for each transcript is $5. ($10 fee for same day requests)

Change Classification Form [Download PDF]

To change a current student's classification from Freshman to Sophomore, Sophomore to Junior, or Junior to Senior based upon the number of credits the student has earned. This requires verification from the Office of the Registrar and the student's signature.

Request to Transfer a Course to ENC [Download PDF]

You must provide the Registrar’s Office with a printed course description from the college catalog of the institution and/or the course syllabus if requested.

Add or Drop a Course/Change of Course Form [Download PDF]

To add or drop a course after the registration period has ended for a semester you must fill out a Change of Course Form. You must have the signature of the professor as well as the signature of your advisor. For every add and every drop of a course there is a $10 fee.

Change of Major Form [Download PDF]

To change or add a major, 2nd major, change your track within your major or add a minor you must fill out a Change of Major Form.

Petition Form [Download PDF]

To petition for a directed or independent study, course overload, course incomplete, change in final exam schedule, or other general academic issue you must petition the Academic Standing Committee (ASC). Request for an Official Letter Form [Download PDF]

To request a letter for various purposes related to enrollment or academic issues you need to complete and sign out a Request for an Official Letter form and submit it to the Registrar's Office. This permits us to release information requested to the receiving party.

Undergraduate Degree Candidacy Form [Download PDF]

To officially notify the Registrar's Office of graduation intent and to provide general graduation information required by the Registrar's Office. You must submit the Degree Candidacy Form along with a completed degree audit - both signed by the student and advisor no later than November 1st of the student's Junior year.

Graduate Programs Degree Candidacy Form [Download PDF]

To officially notify the Registrar's Office of graduation intent and to provide general graduation information required by the Registrar's Office. You must submit the Graduate Degree Candidacy Form no later than November 1st for February graduation and no later than February 1st for May/August graduation.
